TLR4 adjuvant drives early hookworm protection

A phase II trial of NAGST1 hookworm vaccine on L-hydrogel tested three adjuvant strategies in naïve adults using controlled infection. Only NAGST1+CPG (TLR9 agonist) sharply reduced tissue-stage migration and egg output, selecting it for further study.

Eosinophils imply the vaccine blocks migration

Lower eosinophil peaks in NAGST1+CPG suggest the immune response curtails the early tissue invasion stage, before intestine establishment and egg production; the near-zero fecal egg counts then match that mechanism.

Field efficacy may fail without post-infection window

Because helminths often induce immune tolerance and natural infections don’t generate durable protective immunity, performance in already-infected, endemic populations may drop; a likely next design is treating first, then vaccinating.
